What are the options for full arch rehabilitation?
How dental implants generally work, their advantages and disadvantages have already been discussed in our comprehensive guide to dental implants. Now, let's take a closer look at the topic of full arch rehabilitation.
If all teeth in the upper or lower jaw need to be replaced, it is possible to completely replace lost teeth using several implants (usually 6 in the upper jaw and 4 in the lower jaw). This solution is a suitable alternative to a total removable denture.
We can proceed with this if the patient has an adequate amount of bone in the upper and lower jaw, or if augmentation procedures can be performed to create sufficient bone. This process is similar to placing one or more implants, however, there are two variants:
1. Edentulous Jaw: Fixed Bridge on Implants
Phase 0: Extraction of Remaining Teeth
This refers to teeth affected by decay or unsuitable in their position, retained roots, etc. In this initial phase, we extract all remaining teeth or roots and allow the bone to heal (usually 2-3 months). During this healing period, a removable denture is made for the patient.
Phase 1: Implant Placement
Before the procedure, a bone evaluation is performed based on a CT scan to determine the bone quality where the implant will be placed. In cases of significant bone deficiency, it is necessary to augment the bone (using bone substitute material) several months in advance, although it is often supplemented during the implant placement itself. The procedure is performed under local anesthesia.
Healing – the implant integrates with your bone
Healing then follows for 2 to 3 months (depending on the implant type). During this healing period, a process called osseointegration occurs, where the surface of the implants is colonized by the body's own bone cells, and the implant literally fuses with the bone.
Phase 2: Bridge Fabrication
After successful osseointegration, an impression of the implant positions is taken. This is followed by several appointments (3 to 4), and the work is usually completed within 2-3 weeks.
All-ceramic bridge with titanium framework
The bridge consists of abutments and crowns (pontics). It is screwed directly onto the implants, meaning that if necessary, the prosthetic work can be removed and adjusted at any time.

Branemark Bridge

2. Edentulous jaw: Removable prosthesis stabilized on implants
An implant-supported prosthesis is a solution for situations where a patient has lost all teeth in their upper or lower jaw, or when their existing complete denture loses its retention due to progressive bone tissue loss.
Phase 1: Implant Placement

In cases of significant bone deficiency, it is necessary to augment the bone (using bone substitute material), sometimes several months in advance, though it is often done during the implant placement itself. The procedure is performed under local anesthesia.
Hybrid removable prosthesis supported by implants (4 in the upper jaw / 2 to 4 in the lower jaw)

A retention system (locators or bar) is fabricated on the implants, followed by the creation of the definitive prosthesis.
The denture support is based on the MATRIX-PATRIX principle.
The denture is removable, meaning the patient can take it out to clean it and then reinsert it. The denture is only supported by implants to ensure better stability. Chewing pressure is transmitted through the gums.
